Is 12mm a typo?

Assuming it should be 15 and 22mm guides and not 12 and 15?

No, just 15mm and 22mm as these are the only sizes it bends

when using the hilmor do the 30-45 & 90 deg markers finish in middle of tube or at the side for an accurate bend

The markers would finish where ever you chose to pull the bend on the copper pipe. You can choose yourself where to stop the bend. As a rule of thumb I always finish in the centre of the tube for an accurate bend. The degree markers are pre stamped on pipe benders.
